Chronic Condition

Simply put, prevention is better than treatment.

The costs associated with the management of chronic conditions such as hypertension and diabetes are staggering, to say nothing of the lifestyle challenges associated with the diseases.

HealthIO believes prevention is the best medicine and has designed a platform enabling organizations to stratify members based on their risk of developing chronic conditions. Using predictive analytics and user-collected data, HealthIO gives organizations and their members a new tool to help manage health outcomes and healthcare expenditure.

Hypertension Prevention

Hypertension, or high blood pressure, is a symptomless “silent killer” that damages blood vessels and leads to life-threatening conditions like heart attack or stroke. The good news is that hypertension is treatable and often preventable through early diagnosis and heart healthy lifestyle changes. Since there are no symptoms, the only way to find out if you have hypertension is to check your blood pressure regularly.

Maintaining an awareness of your numbers can alert you to any changes and help detect patterns. Tracking your results over time will also reveal is the changes you’ve made are working. By living a heart-healthy lifestyle, you can keep your blood pressure in a healthy range and prevent or delay hypertension. A heart-healthy lifestyle includes: eating a healthy diet full of fresh fruits and vegetables; maintaining a healthy weight or BMI; enjoying regular exercise; limiting alcohol consumption; not smoking; prioritizing sleep and managing stress.

Diabetes Prevention

Diabetes prevention is best accomplished by monitoring blood sugar levels, healthy eating, and staying physically active. Before someone is diagnosed with Type 2 diabetes, they usually have prediabetes, meaning their blood sugar is higher than normal but not high enough for a diabetes diagnosis. Prediabetes is extremely common. In fact, more than 88 million U.S. adults are prediabetic, although more than 84% don’t realize it.

Because prediabetes is often symptomless, it is important to check your blood sugar levels regularly. As with any other health vital, knowing your levels is key to understanding your individual health. One last point of good news - prediabetes is reversible through lifestyle interventions such as maintaining a healthy weight, exercising regularly, and following a healthy eating plan.

Why is this needed?

Nearly half of U.S. adults have hypertension, but only 25% have their condition under control. Hypertension increases the risk for heart disease and stroke. It usually has no symptoms and, hence, regular measurement of blood pressure and assessment of risk becomes very important.

Diabetes Type 2 results from a combination of resistance to insulin and insufficient insulin production in our bodies. 10+% of our adult population suffers from diabetes, however, effective management and therapy can prevent or delay diabetic complications.

Who is this for?

Whether an individual has been recently diagnosed as hypertensive or diabetic, or has been struggling to manage their condition for some time, we focus on using data to individualize the program for more effective management of the condition and peace of mind for everyone (member, family, care givers).

How do we do it?

HealthIO has been built to engage, educate, and empower the individual healthcare consumer, and to give you the tools to actively manage your employees’, members’ or patients’ health outcomes.

Through the HealthIO Chronic Condition Management Program, HealthIO users are provided the tools to easily monitor and track their vitals, and better understand impact or absence of specific behaviors. They receive the necessary coaching and support to stay engaged with their health and make small changes to create new habits and a pathway to long-term engagement with their health.

HealthIO is available anytime, anywhere on any smartphone / tablet and makes it convenient to routinely complete the health “check-ins” that  generate the data and become the foundation of each member's mobile dashboard.

Connected devices

Easy-to-use Bluetooth connected devices make sure that the data required for an effective health check-in is easily generated and totally secure. We deploy everything needed including blood pressure instrument, weighing scale, pulse oximeter, and glucometer – we are holistic, after all.

Individualized coaching and support groups

The complex conversation regarding “health” doesn’t have to be just with a physician. Sometimes it takes a village. HealthIO understands that and brings together those with a vested interest in the individual’s health to participate and increase accountability and motivation. HealthIO creates secure and private individualized support groups and data-led coaching programs to help manage health issues.

Personalized insights

You can't manage what you don’t measure, and it’s difficult to manage what you can’t visualize. Our members learn from their data, with daily insights and visualization that helps them understand their actions and the outcomes they influence.

HealthIO not only provides insights to the individuals, but also the care team – be it the onsite health center, care coordination team or the individual’s care provider. Using data-rich insights specific to each individual, the care team is in a much better situation to provide timely interventions.
